Position Title: LPN 1, 2, or 3 Community Health Worker – PHP
Paygrade: F, G, H
Location: Negotiable Within District
Description of Duties: Under nursing supervision, provides home visitation services to medically diagnosed high-risk, pregnant, post-partum and inter-conceptual women. Works directly with a nurse case manager and other public health nurses to provide needed services and resources to this population. As a community health worker, provides outreach, support and education to the target population and their surrounding community in order to reduce disparities in birth outcomes. May also provide general clerical support. Must have a valid driver’s license and reliable transportation.
Minimum Qualifications: Vocational/Technical degree in licensed practical nursing from an accredited college AND current Georgia license to practice as a Licensed Practical Nurse AND CPR certification AND completion of all necessary orientation and training for Public Health LPN.
Preferred Qualifications: Bilingual-Spanish. Appropriate interventions for high-risk prenatal patients. Appropriate interventions for high-risk newborns and infants. Home visitation experience. One year of Labor & Delivery experience OR One year working in a prenatal care clinic or One year experience working with moms and babies.
