The Perinatal Health Partnership (PHP) Program provides home visiting services during pregnancy and up to the infant’s first birthday following delivery. Pregnant women with high-risk conditions, or who have identified risk factors that put them at risk for poor pregnancy outcomes, will be targeted for home visiting services and interventions but enrollment can occur after delivery.
PHP provides additional monitoring between provider appointments that will increase the opportunity to identify warning signs, complications, and problems earlier and refer to the provider for further assessment and intervention when indicated.
Perinatal Health Partnership services include:
- Clinical assessment of mother and baby
- Care coordination
- Case management
- Education for high-risk pregnant women and their families
- Linkages to needed resources

These services are available at no cost to the client.
